I completed my community rotation with Felicia Porrazza, founder of Porrazza Nutrition LLC in Philadelphia, PA. Due to COVID-19 I did my work remotely. My primary goal was to help clients manage and control their Type 2 Diabetes. Learn more about this in Module 1, which is the introduction to my site!

Module 1 - Introduction
Porrazza Nutrition, LLC
Porrazza Nutrition has grown and flourished over time. Although Felicia does not have a mission statement she believes, "at Porrazza Nutrition, we are all about helping clients create realistic wellness solutions that will build a foundation of healthy eating and exercise habits”. Porrazza Nutrition is all about bringing accessible wellness solutions to clients through virtual nutrition and fitness services. With virtual sessions, Felicia connects with clients through phone or video-secured conferencing sessions. Porrazza Nutrition focuses on giving tools to eating healthy versus a one-size-fits-all diet plan approach. Felicia is all about building a foundation of healthy eating and exercise behaviors that leaves the client feeling satisfied and on track with their goals. At my rotation, I worked to increase Type 2 Diabetes management and awareness. My goal was to educate community members on what Type 2 Diabetes is, how to diagnose it, and how to manage it. I based my health promotion project on Type 2 Diabetes and more information is located on Module 4. Below is my introduction presentation about my community rotation.
Module 2: Ethics & Professional Development
Professional Development Plan
I completed a student professional development plan (previously known as "Goal Wizard") by going through the CDR's 2021 Essential Practice Competencies PDF. My plan includes three goals that are associated with a competency. For each competency there are three performance indicators. Over the course of my dietetic internship I will be working on completing these goals and competencies.

Module 4: Health Promotion & Wellness
My Health Promotion Project consisted of a "Diabetes 101" webinar. The primary goals of the “Diabetes 101” webinar were by the end of the presentation the participants would be able to: define what diabetes is, describe the ABCs of diabetes, demonstrate healthy eating habits while having diabetes, and list examples of physical activity. These objectives were clearly stated on a PowerPoint presentation slide. Overall, completing these objectives would increase participant’s awareness and knowledge on how to manage diabetes while also providing a brief overview on how to create a healthy lifestyle. To reach my goals and objectives I used specific questions on a pre and post test as well as specific marketing strategies to advertise “Diabetes 101”. I then had to create educational activities. Sticking to the theme of managing Type 2 Diabetes I chose to create a “Take Home Tips” handout as well as a “Log Sheet” to log their progress on test results. Pre and post tests were made prior to the initial newsletter registration email blast to ensure that participants took the test before attending the webinar.
BRFSS Report
I created a Behavioral Risk Factor Surveillance System (BRFSS) health report on the prevalence of Type 2 Diabetes in the County of Philadelphia. I chose this area because this is where my preceptor's private practice is located.
Educational Materials
I created two educational materials. The first handout, "Take Home Tips" is all about managing Type 2 Diabetes at home. It goes through the ABCs as well as physical activity. The second handout, "Type 2 Diabetes Log Sheet" is for participants to log their A1c, Blood Pressure, and Cholesterol results. There is also a section on that handout to create personal wellness goals related to their Type 2 Diabetes Management.
Educational Materials Part 2
SMOG Evaluation
I created educational materials for my "Diabetes 101" webinar to help participants manage and control their Type 2 Diabetes. After conducting a SMOG evaluation I found my "Take Home Tips" handout to be at an eighth grade reading level.

Health Promotion Project Report
I wrote a report on all aspects of my Health Promotion Project (HPP). Included in my report is information on my timeline, marketing materials used, data collection, outcome evaluation, and a program outcomes assessment. Details from my HPP from start to finish can be found below.
Outcome Evaluation Form
I completed an evaluation plan summary before and after my HPP. This summary includes the outcome indicators I used to determine the success of my HPP as well as results of my program.
Group Education Evaluation
My preceptor completed the Group Education Evaluation Form after I presented my HPP. Additional feedback and comments can be seen on the form.
Motivational Interviewing
Due to COVID-19, my community rotation was virtual. Because of this, I virtually participated in mock counseling sessions with my preceptor. Comments and critiques can be found on the evaluation.

Social Media Posts
I worked with my preceptor to create social media posts advertising my "Diabetes 101" HPP. Two posts were posted to the Porrazza Nutrition Instagram and Facebook accounts.

I worked with my preceptor to create a Porrazza Nutrition email blast for all Porrazza Nutrition newsletter participants. The time, date, and information on my "Diabetes 101" HPP was listed.
